Step 1: Read the policies and procedures on this webpage.
Step 2: Submit the Provider Information Form to the Enrichment Coordinator, Amanda Swift.
Fall Deadline for all paperwork and payments is August 1, 2019.

Please complete a Provider Information Form for each class via e-mail.
You may request to run multiple classes/clubs, but availability of space will determine how many classes will be scheduled. The Coordinator will notify you of how and which classes/clubs get onto the schedule.

Are you looking for additional help in the areas of reading, math, or writing for your child for your 3rd, 4th or 5th grader? Are you looking for homework help? During this hour, I will work collectively and one-on-one with students to help them complete their homework and/or develop their skills and increase their capacity & endurance for learning and thinking. Tutoring will be personalized based on parent and student input, focused assessments, and targeted instruction. Instructor: Amanda Swift

There are three ways an individual can come in.
· DPS Community Partner
o Community Partners: outside/community organizations who background check their own staff and provide DPS proof of checks and insurance. Services can have a cost or be free.
· DPS Volunteer
o Volunteers: Community or family volunteers. DPS pays for and runs their backgrounds checks. Services are provided at no costs or payment to volunteer and families do not pay. Volunteer can be any individual (teacher or not).
· Teacher/School Staff
o Teachers: Teachers can get extra duty to put on a club afterschool per union pay rates.
o Other School Staff: School staff can hired as an enrichment instructor and get paid an hourly wage as a DPS employee.

All providers must complete this process, including a background check for any teacher/instructor, prior to being able to teach a class/club at Edison. This process needs to be done one time per school year.
Step 4: Complete the DPS Community Use Form/Facilities Use Application and $25 fee made out to DPS Community Use to Amanda Swift @ Edison. Give completed form to me with check and I will submit on your behalf. (If you had one for the fall- it is good for the 2019-2020 school year.)
Download and complete this facility use form from the DPS website. Submit this form and $25 to the DPS Community Use department or to the Edison Extra Coordinator.

All providers must complete this application prior to being able to teach a class/club at Edison.
This process needs to be done one time per year, but indeed every calendar school year.
Step 5: Fill out and Submit the DPS Volunteer Application form and follow the steps for a criminal background check.
If you are a new provider or an old provider with new instructors or substitutes for Edison this spring, you need to take care of this before August 1st, so it has a month to process. If not your class will not be able to run, because you won't be able to be left alone with This only needs to be done once per three years. Definition: Volunteers with access to children in an unsupervised setting (not in line of sight of DPS staff member) – Volunteers wishing to engage with students on a regular basis. For example, an individual who volunteers to come in on a weekly basis to teach an art class after school in the auditorium without a DPS staff member present in the room. This category is not designed for programs or individuals that charge fees to families. Volunteers must complete an Unsupervised Volunteer Application form. This form is provided by the Principal or their designee to the volunteer. This form is not available on line. Once the Office of Volunteer Services receives the unsupervised volunteer application, an email will be sent to the volunteer with further information about obtaining a fingerprint background check. It is recommended that this category be used sparingly as best practice recommends volunteers are supervised by DPS staff. Requires a fingerprint background check. Read, sign and date in the presence of a witness/DPS staff member and/ or designee 3. DPS staff or designee must witness must check a form of ID before signing DPS Request for Criminal History/Background Check: 1. Fill out personal information including full name, date of birth, last four digits of your social security number*, the corresponding number from the photo ID**, and any available phone numbers. 2. Indicate conviction of felonies or misdemeanors by checking corresponding box. *** * If an individual does not have a social security number, indicate this on the form. ** ID must have a photo, for example driver’s license, passport or student ID, but does not need to be government-issued. Foreign ID’s such as a driver’s license or passport are also acceptable if individual does not have a United States ID. *** A DUI or DWAI and any other misdemeanors must be noted on the form. Any information that appears on the background check that was not indicated on the form may cause a denial from DPS. (Please note that not all offenses necessitate a denial, the decision is made by DPS and is often determined on the basis of case outcome). Step 6: Provide Current Insurance Proof to Amanda.
Liability Insurance: Please be aware that the Community Use department requires each provider to carry Liability Insurance. You must submit this information with the Community Use form.

Step 7: Write two checks for School Use and Edison Coordination. The first check to DPS for 1% of your total students fees for each class and one check to Amanda Swift (coordinator) for 1%, as well.
Providers are charged a fee of 2% of their total cost for their class for their maximum number of students accepted per semester. 1% of this fee is for School Use. 1% of this fee is for coordination of Enrichment classes.
All 7 steps must be completed by the provider no later than August 1 for fall of Dec. 15th for the spring class to be scheduled and open for registration for our families.

In order to provide a quality before and after school enrichment program at Edison, which is in compliance with DPS policies, the following procedures must be followed.
Paperwork and fees:
· All providers must have all their paperwork completed and turned in to Edison’s office/Enrichment Coordinator or DPS departments on the required dates in order to offer a class/club at Edison.
· Paperwork/fees includes:
o Provider Information Form
o DPS Community Partnership Application or provide an FBI background check for each employee, including sub teachers.
o DPS Community Use form, including Liability Insurance up to $1,000,000
o Check for 2% of total cost for class for maximum number of students written to Edison Elementary
· Providers are “renting” the space at Edison- made out to DPS. Edison is providing coordination, registration, and advertising for the providers- made out to Amanda Swift
· Providers are required to collect payment from students/guardians for their classes.
School Use:
· Providers agree to provide all materials needed for their classes.
· Providers agree to leave the facilities at Edison in a clean, organized manner. If providers damage property or leave the space they use messy, they may be charged up to $35 per hour for cleaning and/or for repair of the property.
· Providers understand that they may NOT store any materials at Edison.
· Providers agree to sign in to the Edison office 15 minutes before their class begins and wear a Visitor sticker while in the building.
· Providers agree to adhere to Edison’s parking policies.
· Providers agree to maintain their own roster including contact information.
· If providers need to cancel a class, they will contact the school AND parents/guardians.
· For afternoon classes/clubs, providers agree to pick up all ECE/K students at their classrooms and all 1st-5th grade students outside on the playground, on the library steps, at 3:30. Providers agree to release students to their guardians in front of the school. If parents/guardians are missing/late, the provider is responsible for the student until the parent/guardian arrives. If students are in Kaleidoscope, providers must walk these students to the cafeteria after class.
· For morning classes/clubs, parents will escort students to the provider. When the class/club is over, the provider must walk ECE/K students to their classroom/playground/teacher and all 1st-5th grade students to the playground at 8:30.
Registration Periods:
· Edison offers Edison Extra Enrichment classes each Semester (August-December and January-May).
· Providers understand that they are required to complete the Application process each semester, including the fee.
